Day/Date: Friday, May 7th, 2010
Where: Goldsboro Municipal Golf Course, Goldsboro, NC
Timing: Registration from 12:00pm-12:45pm; Shotgun start at 1:00pm
Teams: 4-person teams. Players may sign up individually or as 4-person team.
Format: 4-person Captains Choice (Superball).
Team Entry: $200/team includes beverages and snacks during tournament, and ceremony after golf.
Individual Entry: $50/Player includes beverages and snacks during tournament, and ceremony after golf.
Reception Only: $10.00 to join us for the reception and the ceremony after golf
Donations: I cannot play but wish to make a donation of $_________ (fill out form below)
After Golf: Awards ceremony after play. Prizes for 1st,2nd and last place. Other prizes include closest to the pin, longest drive and longest putt.
To Benefit: All Profits will benefit PULSE of Wayne County
